On the following Pressure injury you are to do
skin and wound assessment (using MEASURE wound assessment tool) (wound assessment may be presented as a table)
briefly outline the aetiology of the wound and any factors which may impact on healing,
a management plan identifying short-term patient and wound goals,
a suitable wound bed preparation regime (e.g. cleansing, debridement if needed, dressing choice, skin care, including frequency of dressing changes required, how long can the dressing product be used)
providing a rationale for choice of dressing(s) or therapy
a discussion of one alternative dressing or therapy option from a different dressing category that could have been used (e.g. this might be a choice that you couldn’t make because the product was unavailable to you) and one inappropriate dressing choice including why it was not suitable for this patient.
